This form should be completed if you wish to request to take a course and you have not taken the prerequisite course(s) first.  The completed form must first be submitted to your advisor for review and then it will be forwarded to the instructor for you. The Director will receive it last.   If it receives approval from all three, then you will be given an override to register or waitlist for the course.

If you wish to change your academic advisor, you will need to complete this form and meet with the faculty member you wish to have as your new advisor.

Complete this form prior to enrolling in pass/fail courses or workshops to be taken outside of SLIS
