Graduation Application

 

Applications for graduation must be submitted by the end of the first week of the semester you wish to graduate. The application form may be obtained by contacting the Kent or Columbus office by emailing This e-mail address is being protected from spam bots, you need JavaScript enabled to view it to request a PDF version. An instruction sheet will be included which details where the forms need to be returned.
